SCHOOL ENTRY IMMUNIZATIONS
Required
ALL NEW TK-6 STUDENTS
What's Required?
Vaccinations are mandatory for all students attending a California public school.
Required vaccines include: Polio (x4), DTaP (x5), Hep B (x3), MMR (x2), and Varicella (x2).
Submitting Your Child's Records
Please submit a copy of your child's complete immunization record during the Pre-Registration process.
Records may be emailed, attached to the Pre-Registration Packet, or uploaded into the Pre-Registration application.
